Equipment Rental Agreement
When equipment is rented from Shan Medical Equipment, Inc., the customer agrees to pay a periodic rental fee for the product(s) rented. It is the responsibility of Shan Medical Equipment, Inc., to service and maintain the equipment for the entire rental agreement period.
Rental agreements can be terminated by renter/purchaser if discontinued by physician or if the patient/resident expires. Shan Medical Equipment, Inc. may terminate a rental agreement for monthly non-payment of the rental agreement fee.
All rentals will be for the stated rental period and no pro-rated adjustments will be made for the unused portion of the rental period.
The Customer agrees to be responsible to either return the equipment to Shan Medical Equipment, Inc., or to be responsible for payment of said equipment if Medicare or your insurance company denies payment.
MAINTENANCE - We will provide you with a schedule for preventive maintenance, repairs and testing of equipment and/or devices as recommended by the manufacturer(s) and will provide a replacement when maintenance requires removal of the item from the home.
Manufactures Warranty- We will be able to work on most brands of equipment that require manufacture warranty work just inquire within.
Replacement- In the event that a replacement piece of equipment is needed we will arrange for the replacement to take place as soon as possible in order to not allow for the patient to be without the equipment for a prolonged period of time.
